General Description/Position Summary:

People who are food and nutrition insecure are disproportionately affected by diet-related chronic diseases such as diabetes and high blood pressure as well as many other adverse effects to overall health. The Community Healthcare Programs Coordinator is an innovative and creative leader who advances the St. Louis Area Foodbank's vision of a nutritional secure Illinois and Missouri resulting in a healthier bi-state region. This person will develop, implement, and measure the success of the Foodbank's programs in healthcare settings throughout our 26-county region with a focus toward meeting immediate food and resource needs, establishing replicable best practices and ultimately, ending hunger and improving health. Additional expectations include serving as the Subject Matter Expert (SME) for external partners, piloting new initiatives and ensuring continuous program improvement. This position represents a critical step for properly addressing health among our food insecure neighbors and helping them live healthier, more stable, and self-sustaining lives.

Objectives:

  • Lead the organizational vision for the Foodbank's programs in healthcare settings.
  • Stay abreast of trends and research regarding best practices in hunger relief, nutrition security, and ending hunger in community healthcare settings.
  • As SME, develop and ensure program plans, logic models and tools are up to date.
  • Develop and provide training and support to partners and team members.
  • Pilot, measure, evaluate, set goals, and ensure the continuous improvement of all community healthcare programs.

Responsibilities:

  • Lead the development and expansion of the Foodbank's community healthcare strategies through the incorporation of food and nutrition insecurity screenings in healthcare settings.
  • Oversee and serve as SME for Healthcare Markets.
    • In collaboration with healthcare partners, use community feedback, needs assessments and data to determine gaps and innovate, pilot and test new initiatives to address them.
    • Ensure replicable program plans, logic models and tools are created and up to date with the latest trends and best practices.
    • Measure the success of all Community Healthcare programs and services, ensure they are meeting intended goals and take steps to continuously improve while ensuring HIPPA requirements are met.
    • Incorporate volunteers, interns and AmeriCorps members into all program models and develop standardized orientation, training, and supervision tools so that partners have a roadmap to support them.
    • Serve as internal SME for Community Healthcare programs with a focus on educating and training team members, interns and volunteers on the needs, programmatic goals, policies, and procedures.
    • Partner across departments to collaborate on inclusive program models and ensure the availability of products and services to meet the needs of Community Healthcare programs.
    • Work cross functionally to identify outcomes and develop metrics to support and reinforce philanthropic and marketing efforts to ensure program models are resourced appropriately. Responsible for ensuring all grant requirements are met.
    • Assist in executing and updating annual department work plan, including budgets, calendars, goals, and tasks.
    • Partner with departmental leadership to craft, refine, and execute the annual department plan.
    • Work in partnership with the Nutrition and SNAP programs, including collaborating on curricula development, outreach plans, implementation, and evaluation in health care settings.
    • Ensure program operations and activities adhere to legal guidelines and internal policies.
    • Represent SLAFB in the community at events, committees and through networking, public relations and speaking engagements including all partnerships and committees related to social determinants of health.
    • Provide oversight and direction to volunteers, interns, and AmeriCorps members.
    • Culture champion who models our core values.
    • Attend program-related training conferences and workshops as appropriate.
  • Perform other duties as assigned.

Qualifications:

    • Bachelors' Degree from an accredited university required. Master's degree with an emphasis in Non-profit Administration, Dietetics, Public Health, Education, or another social service-related field preferred.
    • Experience working in a health care environment and ability to understand health care jargon a plus.
    • 2-5 years previous experience in program management with partners and volunteers of varying socio-economic backgrounds for a non-profit organization preferably in a health and wellness environment.
    • Bilingual a plus.
    • Experience managing grants and meeting set deadlines and deliverables.
    • Prior experience with developing, writing, and executing programs, training and evaluation strategies strongly preferred.

  • Proficient with Outlook, Word, Excel, PowerPoint and other web-based applications. Experience with or the ability to quickly learn Tableau or other mapping software required.
  • Ability to think strategically while acting tactically.
  • Ability to establish and maintain effective working relationships with diverse stakeholders.
  • Ability to work independently and as part of a team.
  • Must have reliable transportation with proof of insurance and be willing to travel within a 125-mile radius.
  • Must be able to pass drug screening and criminal background check.

WORK SCHEDULE: The Community Healthcare Programs Coordinator will work a total of 40 hours per week. The hours are Monday - Friday between the hours of 8:00AM - 4:30PM. This role may be hybrid with work remotely, on onsite at the Foodbank campus, and in the community as needed. However, flexibility will be granted as appropriate.

Occasional weekend (Saturday/Sunday) or evening hours may be required.

Meeting attendance may require overnight travel.

PHYSICAL REQUIREMENTS:

The Community Healthcare Programs Coordinator will spend significant time in the community, at healthcare sites and campuses, in our warehouse and at food distributions. Requirements include:

  • Walking to and from the corporate office to the warehouse, both of which are on the Foodbank campus.
  • Walking on hard concrete surfaces (in the warehouse or in the community).
  • Ability to lift, bend, stoop, pick up and move objects up to 40-50 pounds.
